Insurance

We accept many types of health insurance including, auto insurance, work comp, many HMO and PPO plans, group insurance, and traditional insurance.  To find out if your particular insurance company/plan covers care at our office, call the customer service number on your insurance card.  We do not pre-verify insurance benefits prior to your first visit.  If your insurance company covers services at our office we will bill your insurance at your request.  Patients are responsible for any co-payment at the time of service.
